  This position will work in the department of Lifestyle Medicine, primarily at the North College location serving the Pulmonary Rehab and POTS Rehab Clinic.

    • We use a case management model and each clinician is responsible for care coordination of their assigned class/group of patients during their rehabilitation course.
      
    • Our team is multidisciplinary, including Medical Director, Registered Nurses, Exercise Physiologists, Registered Dietitian, Respiratory Therapist, Physical Therapist, Social Workers, and Patient Access Specialists.
